- Welfare Fund Major Medical supplement ($2,000 annual deductible $1,000 if you buy
the city rider).
- HIP/HMO and HIP CHOICE

Prescription Drugs, Appliances and Private Duty Nursing
- Additional mental health benefits, covers $25 co-pay per visit for HIP/HMO and HIP
Choice Plus
- PHS, Choice Care, Blue Choice

Audiology and Hearing Aid Benefit
- The Audiology benefit is provided at the DC 37 Health and Security Plan Audiology
Center only, which is located at 125 Barclay Street (Room 770), New York, NY, (212)
341-4790. The benefit is available to members only.
