Subject:            Change of Home Address


We are rapidly approaching the end of the calendar year and it is essential that employees report changes of home addresses that have occurred this year to the Human Resources Office in preparation for the production of W-2 Forms for the 2020 calendar year. If you have had a change of home address or anticipate having a change of home address before the end of this calendar year, please be certain to notify the Human Resources Office by completing and signing a Change of Employee Address Form. This form is available on the Kingsborough web site in the Human Resources section under Downloadable forms. For security’s sake, please make certain to use your Reference Number (found on your pay stubs) on the form and not your Social Security Number.

Please be advised that revisions to an employee's home address will only be made upon receipt of WRITTEN notification of a change of address.

IMPORTANT: If you have made a change of home address on the CUNYFirst system via employee self-service, it is still necessary for you to complete a Change of Address Form for the Human Resources Office so that we may confirm that all changes have successfully made their way to the New York City payroll system.  

Please email Change of Address Forms to no later than Tuesday, December 8, 2020 in order to provide sufficient time to process revisions on the payroll system and to ensure that the correct home address will be on the payroll system when W-2 Forms are produced in January 2021
