Instructions for Completing IRB Consent Forms and Sample Consent Forms

The attached sample consent form is provided to assist faculty and students.  It is intended only to be a guide; however, all headings contained in the sample form must be addressed.   If a particular heading does not apply, please provide a statement indicating the reason.  For example, if participants are not compensated for their participation, the statement that “participants will not be compensated” will suffice.

Please note that obtaining completed consent forms from potential participants is a process. Adequate time should be allowed to review the consent form with all potential participants and answers to all questions asked should be provided.  If your study involves adults, your consent form needs to include a statement for participants to acknowledge they are at least 18 years of age.  If your research subjects are minors, parental permission is required.  Parental permission is treated in much the same way as informed consent. 

If the IRB determines that a research protocol is designed for conditions or for a subject population for which permission is not a reasonable requirement to protect the subjects, it may waive the consent requirements, provided certain substitute protections are adequate and the waiver is not inconsistent with federal, state or local law.
